UPIKE Immunization Form

Immunizations (Types of Vaccine Shots)

All incoming international UPIKE students must receive the following vaccinations prior to their arrival on campus:

  • MMR (Measels, Mumps & Rubella)
  • Tetanus-Diptheria-Pertussis
  • Varicella (Chicken Pox)
  • Meningococcal Conjugate (ACWY)
  • Hepatitis B

Recommended Immunizations:

  • Influzena (annually)
  • Covid-19
  • Papilloma (HPV)

All international students are required to be screened for tuberculosis in the United States within six months prior to their first semester. A photocopy of the following will serve as proof of screening:

  • Laboratory report of a negative Quantiferon blood test or T-Spot blood test. Only laboratory test results will be accepted, no handwritten results.            

Please note: A history of BCG immunization does NOT fulfill this requirement.
